One Pager – Provider MIPS Measures
A One-page reference guide to the measure required to be collected during a visit to assist with satisfying the MIPS 2023 measure requirements. These measures must be completed, or verified that the measure was previously completed, on all patients, all office visits*. The requirements of each measure are listed below as a quick reference guide and more in depth in subsequent pages
*Office visits include Office E&M services only (99202 – 99215)
You will be able to utilize the MIPS 2023 Checklist page to determine which measures are needed to be completed by you. The MIPS 2023 Checklist will be completed by the Intake team and provided to you after intake has been completed. Items included in the MIPS 2023 Checklist are in yellow
Every Visit:

Medications (Measure 130): For all medications listed in the clipboard and/or prescribed during the current visit must include:
- Route
- Dose
- Strength
- Frequency
- Non-Use of High Risk Meds (Measure 238) in the elderly is easily met each encounter by not prescribing 2+ high risk meds to the patient.
- Weight Counseling (Measure 128): Counseling patients with an abnormal BMI
- Unhealthy Alcohol Use (Measure 431): Counseling patients with unhealthy alcohol use
Once a Reporting Period:
- Screening for Osteoporosis in Women 65+ (Measure 39): Documenting if DEXA results are available

Tobacco Cessation (Measure 226)

Any patient listed as a Current tobacco user in the Social History section of the clipboard needs cessation counseling (Current Visit or w/in last 12 months)
- Document “Counseling – Tobacco Use”
- Document “Smoking Cessation Counseling” to bill 99406 for 3+ minute counseling
EMA Quality Measure Notes:
A patient qualifies for any measure that is listed in the “Quality Measure” section of the Visit:
- Red/Unfavorable notates a patient qualifies for a specific measure, but the measure is not yet met
- Green/Favorable notates the measure is satisfactorily/favorably met for this patient visit.

Any applicable measure is listed here, even measures not currently being reported by our group.
- You must know which measures you are intending to report on during the reporting period.
